Notification Requirements for Confidential Information Breaches For Vendors, Employees, Contractors, or Researchers
State policy requires that vendors, employees, contractors, or researchers report any privacy breaches (e.g. stolen computer equipment with client information on it or client paper records left in a public place). The Agency Security Breach Report (SIMM Form 65C) contains all of the required reporting information. Instructions for filling out the form are located on page 3 & 4 of the form. All sections of the form need to be completed for the form to be considered complete. A signature is not required on the form. The form can be completed in Microsoft Word and emailed to Mark Halvorsen (mhalvorsen@cvrc.org) or printed out and mailed to him at Central Valley Regional Center, 4615 N. Marty, Fresno, CA 93722.
